Membership fees cover select virtual primary care services provided by Amenity Healthcare.

Additional costs may apply for labs, imaging, medications, specialist care, emergency care, hospital care, or services provided by outside organizations.

Services are provided based on clinical appropriateness, provider judgment, state law, and telehealth limitations. Some concerns may require in-person evaluation, urgent care, emergency care, or referral to a specialist.

Visit Use & Monthly Follow-Up Policy

The Amenity Circle™ Membership includes one virtual follow-up visit per month.

Monthly visits are intended to support ongoing care, accountability, medication monitoring when clinically appropriate, and progress toward your health goals.

Unused monthly visits do not roll over, accumulate, or convert into credits, refunds, or additional visits in future months.

If you are unable to attend your monthly visit, we encourage you to reschedule within the same billing month, based on appointment availability.

Membership fees help support continued access to care coordination, secure messaging, chart review, treatment planning, and provider availability, even if no visit is used in a particular month.
