Emergency care or crisis services, hospital care or inpatient services, in-person visits, procedures, or physical exams, imaging, scans, or outside diagnostic testing fees, lab fees charged by outside laboratories prescription medication costs guaranteed access to or prescription of any specific medication, including GLP-1 medications, specialist visits or referrals outside of Amenity Healthcare, bariatric surgery or surgical services, disability, FMLA, legal, school, or administrative forms unless reviewed and approved separately, care provided outside the states where Amenity Healthcare is licensed, services not clinically appropriate for telehealth.The Amenity Circle™: do not allow visit accumulation/rollover.

Membership includes select virtual weight management services provided by Amenity Healthcare.

Additional costs may apply for labs, imaging, prescription medications, specialist care, emergency care, hospital care, or services provided by outside organizations.

Weight loss medications, including GLP-1 treatment options, may be discussed as part of your care plan when clinically appropriate. However, prescribing is based on medical history, clinical appropriateness, provider judgment, state law, medication availability, and safety considerations.

Visit Use & Monthly Follow-Up Policy

The Amenity Circle™ Membership includes one virtual follow-up visit per month.

Monthly visits are intended to support ongoing care, accountability, medication monitoring when clinically appropriate, and progress toward your health goals.

Unused monthly visits do not roll over, accumulate, or convert into credits, refunds, or additional visits in future months.

If you are unable to attend your monthly visit, we encourage you to reschedule within the same billing month, based on appointment availability.

Membership fees help support continued access to care coordination, secure messaging, chart review, treatment planning, and provider availability, even if no visit is used in a particular month.
